Welcome to the St. John's Jesuit HS and Academy Health Office. These web pages will provide you with health information to help care for your son. You will be able to download all necessary medical forms, including emergency, physical, medication, asthma inhaler and allergy sheets.

H1N1 Update:

SJJ will be administering the H1N1 vaccine to students who want it on December 1, 2009. In order to receive the vaccine, please fill out the 2009 H1N1 Influenza Vaccine Consent Form below and return it before December 1 to the Principal's office.
